If you are having photos done by Trivi Photographi we require you to sign and complete this release form. This is just stating that Trivi Photographi has rights to use your images in media. Please fill out this form at your earliest convenience and email it to triviphotographi@yahoo.com. If you are electronically filling out the release form, you may use your initials in case of the signature. Once the release forms have been received, your photos will be released when they are ready.
